Request Sympathy Card and Donation for our Dogs

The Wichita Dog Training Club is like a family and when one of our dogs passes away we as a family feel the loss and grief for our fellow member. This form is to request sending a sympathy card to the member whose dog has passed away and as an option send donation, not to exceed $25.00 to a dog related organization on behalf of the member of the Wichita Dog Training Club to recognize and honor the passing of their dog.
