Enrollment form to obtain student and dog information for Agility Classes By enrolling and submitting this form I agree to the following: I understand my dog and I cannot participate in class without completing, and submitting this form. I understand that my dog is required to be up to date with required vaccinations and that I will be required to submit proof of vaccination at the first class to participate. I understand that all dogs participating should be free of infectious diseases. If my dog or myself become ill, we will not attend class and we will contact our instructor to let them know we will be absent. While understanding that WDTC strives to provide a safe environment to train dogs, I understand that dog training may involve risks to myself, members of my family, or my dog. I assume all risks associated with this training class and will not hold Wichita Dog Training Club, its instructors, its board of directors or its members responsible for any injury, damage or loss to myself, my family members, or my dog. I hereby agree and covenant for myself, heirs and executors and anyone else who may claim on my behalf to waive, release and discharge Wichita Dog Training Club from all claims arising out of or in connection with training associated with said club. I also agree to assume all responsibility for any damage done to property, persons, or other dogs done by me, my family members, or my dog’s actions.
